Forms & Surveys: Export Form Responses

Navigation: Admin > Forms & Surveys

Click Report for the data you wish to view

Click the Gear Icon

Users can include additional data from the student record when exporting Form responses in the Reporting area.

Data includes:

• EL
• SED
• Special Ed
• 504
• Race
• Ethnicity
• Residence Category
• Age
• IEP
• Foster
• Homeless

Schools can use a page policy to default any/all of these checked.

• Options cogwheel > Page Policies > /admin/forms_surveys.php > Additional Data Fields
